Generally a bulge such as the one you describe is due to excessive moisture and/or insufficient anchoring of the underlayment to the subfloor. It's also possible that someone spilled a liquid, such as coffee or soda. If the area is large, you should replace the faulty piece(s). If it's confined to a small area, use 1" screws to attach the underlayment securely to the subfloor. Nails may pop out over time. The last thing you want is a hump in the floor that will transfer through to your finish flooring or cause squeaks. This is definitely something that your contractor should fix for you.
